Consent Codes Window
The Consent Codes window allows you to configure the consents available for use at your institution.
Consent Codes - Field Definitions
Code
|
A text field that indicates the code that identifies the consent.
|
Inactive
|
A check box that, when selected, indicates the field is inactive and not in use.
|
Form Type
|
A set of radio buttons that indicates the type of form used with this code:
- Scanned In: This consent is manually signed by the patient and then scanned into axiUm.
- On-Line with Signature: This consent uses a Crystal Report to display the consent to the patient and can be signed digitally and saved in axiUm.
|
File
|
A text field that indicates the path to the crystal report file used for this consent code.
The ellipsis is used to navigate to the .rpt file and save it to the consent code record.
Note: This field is only enabled if the Form Type field is set to On-Line with Signature.

Section / Sub-tab
|
Drop-down lists that indicate the Attachment Section and sub-tab to save the generated letters for viewing in the Attachments/Consents module.

Patient Signature
|
A drop-down list that indicates the signature requirements for the patient:
Note: This field is only enabled if the Form Type field is set to On-Line with Signature.
|
Default Expiry
|
A text field and drop-down list that indicate the number of days/weeks/months before this consent expires on the patient's record. |
Expiry Required
|
A check box that, when selected, indicates that the Default Expiry field must be entered. |
Treatment Based
|
A check box that, when selected, indicates that this consent code is associated with treatments.
|
Consent can be Denied
|
A check box that, when selected, indicates that the patient can deny consent.
Note: This field is only enabled if the Form Type field is set to On-Line with Signature.

Pass current user to report
|
A check box that, when selected, indicates that the logged in user information should be passed to the Crystal Report consent file.
Note: This field is only enabled if the Form Type field is set to On-Line with Signature.

Customized Denial Text
|
A text field that indicates which identify custom text displays when a patient denies consent.
Note: This field is only enabled if the Consent can be Denied check box is selected.

Patient Initials Window
(Maintenance > EHR > Consent Codes)
The Patient Initials window allows you to specify any pages of the consent that should capture the patients initials, and whether those initials are required. It displays when you open the Consent Codes window, select a listed consent code and click the Patient Initials... button.
Patient Initials - Field Definitions
Consent

A disabled field that indicates the selected consent code.
|
Page Number
|
A text field that indicates the page number of the consent this record applies to.
|
Description
|
A text field that displays to the patient and describes what the initials are for.
|
Initials Required
|
A check box that, when selected, indicates that the patient must initial this page of the consent. |
Patient Initials - Tasks
Add Initial Pages
|
- Click the Clear Data () icon.
- Complete the fields as necessary and click the Add a new Record () icon.
|
Edit Initial Pages
|
- Select a listed entry.
- Make any necessary changes and click the Modify Record () icon.
|
Delete Initial Pages
|
- Select a listed entry and click the Delete Record () icon.
- When prompted, click Yes to confirm the deletion.

Consent Code Signatures Window
(Maintenance > EHR > Consent Codes)
The Consent Code Signatures window allows you to specify the non-patient signatures required for the selected consent code.
Examples: Witnesses, parents or guardians.
It displays when you open the Consent Codes window, select a listed consent code and click the Consent Code Signatures () icon.

Consent

A disabled field that indicates the selected consent.

Name of Person
|
A text field that indicates the person to sign the consent.
|
Required
|
A check box that, when selected, indicates that this signature is required.
|
Name of person must be entered
|
A check box that, when selected, indicates the name of the signing person must be entered. |
Consent Code Signatures - Tasks
Add Signatures
|
- Click the Clear Data () icon.
- Complete the fields as necessary and click the Add a new Record () icon.

Edit Signatures
|
- Select a listed entry.
- Make any necessary changes and click the Modify Record () icon.
|
Delete Signatures
|
- Select a listed entry and click the Delete Record () icon.
- When prompted, click Yes to confirm the deletion.
|
Reorder Signatures
|
- Select a listed entry.
- Click the Up and Down buttons to move the selected entry within the list.

Consent Code Access Window
(Maintenance > EHR > Consent Codes)
The Consent Code Access window allows you to manage access to the selected consent code. It displays when you open the Consent Codes window, select a listed consent code and click the Level Access () icon.
Consent Code Access - Field Definitions
Code
|
A disabled field that indicates the selected consent code.

Level
|
A disabled field that indicates the selected user level(s).
Tip: You can select multiple levels to save time. If multiple are selected, a comma and ellipsis (, ...) display after the first level.

Access
|
A drop-down list that indicates access for the selected user level(s):

Configure Consent Code Access
|

Select one or more user levels that should have the same access.

Set the access fields as necessary and click the Modify Record () icon.

Repeat the steps for all necessary user levels.
