Billing Doctors / Schedules List Window
The Billing Doctors / Schedules List window allows you to search for and manage the list of billing doctors and fee guides (and their associated fee schedules) in axiUm.
Billing Doctors vs Fee Guides:
In order to submit claims to insurance there must be a billing doctor number attached.
Billing doctors (Provider Types that are configured as Doctor (bills under self)), are dentists who have their own Tax ID or license number and use their own information (specialist IDs, billing address, and an electronic signature) on claims. Additionally, they will specify the fee schedule to be used.
Other treating providers, such as dental students, (Provider Types that are configured as Treating Provider (Bills under clinic)), are not (yet) doctors and must bill under an entity/doctor. In this case, a Fee Guide record is created to store the information to be used on claims. Fee schedules are specified for these record types as well.
Note: When adding records, only Fee Guide type records can be created, as the Billing Doctor records are created when the actual user and provider records are created. Both types of records can be reviewed and managed from this window.
Billing Doctors / Schedules List - Field Definitions
Criteria

A text field that is used to enter your search criteria.
Tip: You can enter the first few characters of the word, or enter an underscore (_) to return all results.

Show Deleted
|
A check box that, when selected, will include records that have been deleted.

Show Inactive
|
A check box that, when selected, will include records that have been marked as inactive.

Billing Doctors / Schedules Window
(Maintenance > Office > Billing Doctors / Schedules List)
The Billing Doctors / Schedules window allows you to specify information about billing doctors and fee schedules in axiUm.
It displays when you open the Billing Doctors / Schedules List window and click the Create a new Record () icon or Edit Record () icon.
Reminder: When adding records, only Fee Guide type records can be created, as the Billing Doctor records are created when the actual user and provider records are created. Both types of records can be reviewed and managed from this window.
Billing Doctors / Schedules - Field Definitions
Name Section:
Prov. Type

A drop-down list that indicates the Provider Type for the billing doctor being defined.
Note: You can only create fee guide records from this window. Billing doctor records are created when the provider is added from the Users window.

Billing ID
|
This field indicates the Billing ID to use when this type of provider performs work.

Title
|
A drop-down list that indicates the Title to use when addressing this billing entity.
|
Inactive
|
A check box that, when selected, indicates the code is inactive and not in use.

Last Name / First Name / Middle Name |
These fields indicate the name of the billing entity.

Click this button to display the Provider Signature window, which allows users to capture, review and manage a provider's signature. |
Claim Information Section:
Organization
|
A radio button that, when selected, indicates that the billing entity is an organization and not an individual. |
Provider
|
A radio button that, when selected, indicates that the billing entity is individual (doctor). |
Organization
|
This field only displays if the Organization radio button is selected and indicates the name of the organization. |
Last Name / First Name |
This field only displays if the Provider radio button is selected and indicates the name of the claim doctor.

Address / City / State / Zip / County |
Text fields that indicate the address or location information for billing doctor.
The ellipses are used to display the City List window and State Codes window to assist users in selecting a city and state.

Home Phone
|
Text field that indicates the home phone number of the billing doctor with the area code and phone number entered separately.

Business Phone
|
Text field that indicates the business phone number of the billing doctor with the area code, phone number, and extension entered separately.

Name on Claim
|
A drop-down list that indicates the type of entity to print on the claim form when claims are sent for this billing doctor.
- Billing Entity: Print the provider name and address from this window on the claim form when claims are sent for this billing doctor.
- Practice: Print the organization name and practice address on the claim form when claims are sent for this billing doctor.

Fee Schedule

This field represents the default fee schedule used for treatments with this billing ID.
The ellipsis is used to display the Fee Schedule Codes window, to select the appropriate fee schedule.

|
This field indicates the billing doctor's license number. |
SSN
|
This radio button, when selected, indicates that the field is a Social Security Number.
|
TIN
|
This radio button, when selected, indicates that the field is a Tax Information Number.
|
UPIN-USIN Id
|
This field indicates the billing doctor’s universal EDI submitter ID for EDI claims. |
NPI
|
This field indicates the personal NPI number to print on insurance claims generated for
this billing doctor. This is a 10-digit numeric code. |
NPI (Type 2)
|
This field indicates the organization’s NPI Type 2 number. This is a 10-digit numeric
code.
|
Dental Taxonomy Code
|
This field indicates the HIPAA dental taxonomy code to indicate the billing doctors' field
of specialty on claims. This is used for EDI claims only, and is a 10-digit, alpha-numeric
ID for X-12N format dental claims. |
Medical Taxonomy Code
|
This field indicates the HIPAA medical taxonomy code to indicate the billing doctors' field
of specialty on claims. This is used for EDI claims only, and is a 10-digit alpha-numeric
ID for X-12N format medical claims.

Dental Specialty Code
|
This is used for EDI claims only, and is a three-digit number representing the billing doctor's dental specialty.

Medical Specialty Code
|
This is used for EDI claims only, and is a three-digit number representing the billing doctor's medical specialty.

Add Fee Schedule Codes

- Complete the fields as necessary and click OK.

NPI / Taxonomy Code Mappings Window

The NPI / Taxonomy Code Mappings window allows you to map NPI and Taxonomy codes for use with EDI claims.
It displays when you open the Billing Doctors / Schedules List window and click the NPI / Taxonomy Code Mappings () icon.

NPI

A text field that indicates the NPI number being mapped.

Ins. Company
|
This is an optional field that allows you to indicate a specific insurance company that this mapping applies to.
The ellipsis is used to display the Insurance Companies List window to search for a select the appropriate insurance company.

A text field that indicates the taxonomy code being mapped.

Add NPI / Taxonomy Code Mappings

- Click the Clear Data () icon.
- Complete the fields as necessary and click the Add a new Record () icon.

- Select a listed entry.
- Make any necessary changes and click the Modify Record () icon.

Delete NPI / Taxonomy Code Mappings
|
- Select a listed entry and click the Delete Record () icon.
- When prompted, click Yes to confirm the deletion.
